Facility Use Consent

I hereby acknowledge that I was briefed with, have inspected all equipment and facility, and understood the directions for the use of the Par 5 Golfing LLC facility, equipment, and golf simulator. I acknowledge that I find myself fit and healthy enough to engage in indoor golf activities and lessons and do not suffer from any such medical or other conditions or injuries that may put me at risk or aggravate my condition. I acknowledge that I would not engage in sporting activities if under the influence of medication, alcohol, or other types of substances and drugs. I understand that I may be coming in proximity to other people suffering from infectious diseases such as COVID-19 and they may not disclose or be aware of it. I also understand that during the activities, I may be exposed to the usual risks involved in sporting activities such as losing balance, tripping, dizziness, bouncing ball, being in the range of a golf club, or another type of injury and I am willing to take that risk with full responsibility. I understand the risks assumed due to my age and those under my supervision, such as minors. 

I hereby waive any and all claims, whatsoever, against Par 5 Golfing LLC, its owners, and managers for any of the above mentioned situations.
